Reporting the Death of an American Citizen Abroad

In order to obtain a Report of Death of an American Citizen Abroad, the following documents must be submitted to the Consulate:

 

1. Autopsy Certificate or Medical Certificate of Death from the attending Physician.
2. Death Certificate from Ecuadorian Civil Registry.
3. U.S. Passport (and naturalization certificate, if applicable) of the deceased.
4. Social Security Number of the deceased.
5. Local address of the deceased.
6. U.S. address of the deceased. 
7. Time and place of death.
8. Exact place of burial, if applicable.

Upon receipt of the documents the Consulate will issue a Consular Report of Death Abroad. There is no fee for this service.

TRANSPORTING REMAINS TO THE UNITED STATES

If the body is being transported to the United States for burial, the following are required:

  1. Autopsy Certificate or Medical Certificate of Death from the attending Physician.
  2. Death Certificate from Ecuadorian Civil Registry.
  3. U.S. Passport (and naturalization certificate, if applicable) of the deceased.
  4. Export Permit from the Ecuadorian Ministry of Health.
  5. Name, address and phone number of funeral home at place of destination (consignee).
  6. Flight information and itinerary. Should include name of airline, flight number, date of travel, departure and arrival times, and port of entry.
  7. Certificate of Embalming. (Cremation is available in Ecuador. The requirements to transport cremated remains are the same as the above except that the Certificate of Embalming is replaced by a Certificate of Cremation.)

Upon receipt of these documents the Consulate will prepare the following:

  1. Consular Report of Death Abroad of a U.S. Citizen.
  2. Affidavit Document signed by the funeral home that states that they have prepared the remains for shipment.
  3. Consular Mortuary Certificate. This document is required to allow the remains to enter the United States.
  4. Transit Label.
