Electronic visa applicaction form (EVAF)

Beginning on November 1, 2006, all non-immigrant visa applicants worldwide will be requiered to use an electronic visa application form to apply for U.S. visas.  As of that date, the U.S. Consulate General in Guayaquil will only accept visa application forms that have been filled out over the internet.  Handwritten or typed visa applicaction forms will no longer be accepted.

The electronic visa application form is available at no cost on the webpage of the U.S. Department of State.  It is found at http://evisaforms.state.gov  Instructions are available in many languages (including Spanish), but applicants must complete the form in English. Once you have filled it out online you must print it, including the third page with the bar code, and present all three pages on your interview day.

Use of the electronic visa application form will help expedite the visa interview process and assist visa applicants to properly fill out application forms prior to arriving at the Consulate for their visa interviews.  Persons who do not arrive at the Consulate with an electronically prepared nonimmigrant visa application form (DS-156) will not be admitted for their interview and will be requiered to contact the Visa Information Service Call Center 1-800-010-145 in Ecuador or 1-800-919-5290 in the United States, for a new appointment.  For more information on the Visa Information Service, please click here.
